Submit Opt-In Form

PLEASE TAKE NOTICE that the United States Bankruptcy Court for the District of Delaware recently approved procedures related to a proposed administrative claims settlement (the “Settlement”) with Forever 21, Inc. and its affiliated debtors (collectively, “Forever 21”). All administrative creditors will receive individualized opt-in forms (the “Official Opt-In Form”) whereby they can affirmatively consent to the Settlement. Acceptance of the Settlement may only be effectuated by returning the Official Opt-In Form. The deadline to submit the Official Opt-In Form is May 28, 2021, at 4:00 p.m., prevailing Eastern Time .

SUBMIT AN OPT-IN FORM ELECTRONICALLY

PLEASE TAKE NOTICE that the Official Opt-In Form is being sent to the primary address of holders of all administrative claims. The Official Opt-In Form is the only form that may be used to enter into the Settlement. The Official Opt-in Form may be submitted either electronically via this website or by returning a hard copy in accordance with the instructions set forth on the Official Opt-in Form prior to May 28, 2021, at 4:00 p.m., prevailing Eastern Time .

To submit an Official Opt-in Form electronically, please enter (i) your name, (ii) the email address to which you want Prime Clerk LLC (now known as Kroll Restructuring Administration) to send confirmation of your Official Opt-in submission and (iii) your unique E-Form ID number in the fields below. Your unique E-Form ID number is printed on the Opt-In Form you received in the mail as part of the Official Opt-in Form package.
